Setting up a custom patient agreement

Clinic Licenses have the ability to have a patients/clients who link with a clinician on the license to sign a patient agreement through the app as part of linking. You can then download all the signed agreements through the dashboard.
This is something normally done as part of setting up a clinic license but can be done at any time. You can retrospectively get patient agreements for active patients as well.
Start by downloading our template Patient Agreement for Recovery Record or Nourishly. Then edit to suit your needs. Send this to your assigned clinic implementation manager or support@recoveryrecord.com. We'll take it from there and let you know when it is live.
