You'll be asked for Member ID, First and Last Name and possible Date of Birth. This is used to confirm your client is a UnitedHealthcare Member. You'll then be asked a couple of extra questions like what level of care, virtual vs in-person and whether this patient's treatment is Private Pay or covered by insurance (or a mix).

Once this is done, that link is “Sponsored” by UnitedHealthcare whilst they are still a member and doesn't count toward your paid links.
